Cqc application new provider

Use this service if you:

- want to register as a provider for the first time
- want to change your business type
- want to tell us about a sale and transfer
- registered as a provider in the past for a different business

You must register with the CQC if you want to provide regulated activities.

You can use this service if you are one of the following types of business:

• charity
• limited company
• limited liability partnership (LLP)
• partnership
• public limited company
• sole trader

There is a different service if you want to make changes to:

• the regulated activities you provide
• your registration conditions
• your locations
• the members of your partnerships

If you’re already registered with the CQC, we’ll ask for your provider ID during the application.

Your main point of contact will need to have a Disclosure and Barring Service (DBS) check that is less than 12 months old to continue the application. It can take several weeks to receive a DBS check so we recommend you start that process before you start this application.

You will also need to provide one or more of the following documents:

We’ll ask you to upload these as part of your application according to the types of service you will provide. The file size limit is 125MB for each document.

It can take up to a full day to complete this application.

You should submit your application when you are ready, or very near ready, to start providing your services.

Once we receive it, we will review it and book an inspection date with you. This may take up to 12 weeks from when we receive your application.

We may request more information before we can make a decision.

You could be fined up to £2,500 if you give false or misleading information
